Nebraska Medicine offers three medical plan options, each designed to provide flexibility and comprehensive coverage.

Medical plan options

Choose from three comprehensive medical plans. Each plan is designed to maximize your benefits while encouraging the use of network providers to minimize out-of-pocket expenses. Explore the plan details below to choose what's best for you and your family.

PPO plan

Enjoy the simplicity of copays for office visits and prescriptions. This plan is eligible for a Flexible Spending Account (FSA) for additional savings. Benefit from premium discounts each pay period if you completed your Empower Your Wellbeing incentives.

Consumer Choice Value with Health Savings Account (HSA)

This plan features a higher deductible with lower premiums. Benefit from employer funding to your HSA if you completed your Empower Your Wellbeing incentives.

Consumer Choice Advantage with Health Savings Account (HSA)

Similar to the Value Plan but with a lower annual deductible. Also includes employer funding to your HSA if you completed your Empower Your Wellbeing incentives.

All plans provide access to a broad network of providers and focus on maximizing benefits through in-network care. However, they differ in deductibles, out-of-pocket costs and coverage features.

Which plan is right for me?

Choosing the right medical plan is important for managing your health care costs and coverage needs. We offer three plans to fit different needs and preferences. Watch this video to learn more about your options and find the best fit for you.

The PPO plan is ideal if you prefer lower upfront costs for doctor visits and have higher medical expenses. This plan might be right for you if:

  • You frequently visit specialists or need urgent care. 
  • You prefer lower copays for office visits and urgent care.
  • You’re OK with higher premiums in exchange for lower copays and out-of-pocket maximums.

Note: The PPO plan is not eligible for a Health Savings Account (HSA). With this plan, you're eligible to enroll in a Flexible Spending Account (FSA) for additional savings.

The Consumer Choice Advantage plan offers a balance with lower monthly premiums and the option for a Health Savings Account (HSA). It's a good fit if:

  • You expect moderate medical expenses.
  • You want lower premiums and are OK with higher costs until you meet your deductible.
  • You want to benefit from Nebraska Medicine HSA contributions and use tax savings for future medical expenses.  

The Consumer Choice Value plan offers the lowest monthly premiums with higher deductibles. It may be ideal for you if:

  • You want the lowest premium per paycheck and are comfortable with higher out-of-pocket costs if you need medical care.
  • You use medical services less frequently and are looking for basic coverage.
  • You want to benefit from Nebraska Medicine HSA contributions and use tax savings for future medical expenses.  

Understanding your provider network and costs

Your costs, such as co-insurance and copays, depend on the network tier of your provider. Here's a breakdown of the provider tiers:

This includes Nebraska Medicine, Nebraska Health Partners, OrthoNebraska, Children's Nebraska and Children's Physicians, Boys Town Physicians and others.

This includes Methodist Health Partners, Bryan Health, Fremont Health, Boys Town Hospital, CHI Mental and Behavioral Health Inpatient Facilities and others.

This includes the UnitedHealthcare Choice Plus network.

There is no coverage for out-of-network care that is nonemergent. This includes most CHI providers and facilities. Be sure to use UMR's provider search tool to verify your provider's network tier.

  • Online search: Visit umr.com, register or log in, and use the provider directory. Be sure to search within the Nebraska Medicine Provider Network.
  • Call for assistance: You can also contact customer service at 800.826.9781 for help finding an in-network provider near you. 

Save more with an HSA and HDHP

Our Advantage or Value Consumer Choice plans cover certain preventive care at no cost. Contribute tax-free to an HSA and receive up to $1,500 in employer contributions. After reaching your out-of-pocket max, your plan covers 100% of eligible expenses.

Expanded coverage for online therapy

Starting Jan. 1, 2025, Talkspace will be available under Tier 1 network coverage for all Nebraska Medicine medical plans. For Consumer Choice plans, therapy sessions are covered 100% after you meet your deductible, while the PPO plan has a $30 copay.  Both therapy and psychiatry services are available – therapy for individuals 13 and older and psychiatry for those 18 and above. Connect with a licensed therapist or psychiatrist from home on your schedule.
